Parade Entry Form

This year, we’re focusing on the idea of “HOMEGROWN.” And there are lots of

Fun Day Sprint

Come RUN in the North Yarmouth Fun Day Sprints. Register for the 2009 Fun Day Sprints before September 4th and receive a reduced regisration fee.

Entertainer Registration Form

This form covers the agreement between NYEC and its performers. The form can be completed and submitted online. Download

Fun Day Supporter/Advertiser

Use this form to register to become a Fun Day supporter by advertising in our Fun Day brochure. There are various levels of support, all of which are explained on the advertising form. Download

Fun Day Table Rental Request

This form is used to request a table or booth at the Annual Fun Day event. It should be completed and returned to the committee member whose name appears at the bottom of the form. Download

Volunteer Sign-up Form

This form may be used to sign up as a volunteer for Fun Day. Anyone who signs up on or before August 10, 2009 will receive a FREE Fun Day 2009 t-shirt. We encourage you to contact any one of the nonprofits if you wish to volunteer for a food booth or to staff one of their table during the event. The form can be submitted online or printed and mailed. Download
