This is a list of forms used by NYEC. Looking to volunteer, rent a table, perform or enter the parade. You can download the form here. Fun Day Table Rental Request (Deadline July 1)

This form is used to request space or a table at the Annual Fun Day event. It should be completed and returned to the committee member whose name appears at the bottom of the form. Download

Cribbage Tournament (co-hosted by Bud Elston)

Proceeds of the cribbage tournament will benefit a local citizen in need. Fee is $10 per person. Download the information brochure and registeration form.

Diaper Derby (co-hosted by Rose Garden Preschool)

Annual diaper derby. Trophies to first place in each category. First 30 registratants will receive a free t-shirt (childs XSmall). Fee $5 per child. Download the information brochure and registration form.

Horseshoe Tournament (co-hosted by Rob Wood)

Annual horseshoe tournament. Ribbons given to First, Second and Third Place Team. Fee is $5 per player/$10 per team. Download the information brochure and registration form.

North Yarmouth Distinguished Citizen Award (Deadline July 1)

An annual award given to a citizen of North Yarmouth who regularly volunteers for the betterment of our community. Download nomination form or contact Jason Raven at mr.corvus@gmail.com for more information.

Entertainer Registration Form

This form covers the agreement between NYEC and its performers. The form can be completed and submitted online. Download

Fun Day Supporter/Advertiser

Use this form to register to become a Fun Day supporter by advertising in our Fun Day brochure. There are various levels of support, all of which are explained on the advertising form. Sponsor Form Nonprofit Sponsor Form

Volunteer Sign-up Form

This form may be used to sign up as a volunteer for Fun Day. Anyone who signs up on or before July 30, 2011 will receive a FREE Fun Day t-shirt. We encourage you to contact any one of the nonprofits if you wish to volunteer for a food booth or to staff one of their table during the event. The form can be submitted online or printed and mailed. Download
